The highly acclaimed Institute of Medicine Report, Dying In America, advises that frequent conversations should occur between clinicians and patients to ensure that patients receive the care they want and avoid treatment they do not desire. This includes conversations regarding values, end-of-life goals, and informed preferences on options available. The report concludes that these conversations should be initiated by clinicians. Honoring Choices® Tennessee has compiled the resources below for providers and healthcare administrators.

– A POST is an important document used in end-of-life planning to allow patients with serious illness or frailty to record their choices for medical treatment. It is medical order for care, not an advance directive.
